Plan-by-plan breakdown
PALS (Pediatric Advanced Life Support) certification in 2026 is required for nurses, physicians, paramedics, and other healthcare professionals working in pediatric intensive care units, pediatric emergency departments, neonatal ICUs, and other pediatric critical care settings. PALS builds on BLS and is the pediatric equivalent of ACLS for adult patients.
AHA HeartCode PALS blended learning is the gold standard PALS certification path. The HeartCode PALS online at $178 covers cognitive learning via simulation scenarios tailored to pediatric emergencies: respiratory distress, shock, arrhythmias, post-cardiac arrest care, and toxicology. The required in-person skills session ($60-$110) covers hands-on pediatric megacode practice with AHA PALS instructors.
The combined AHA-aligned cost is $238-$288. This is the path accepted by virtually all US pediatric hospitals, pediatric ICUs, pediatric ERs, and large hospital systems. No employer in major US pediatric healthcare settings rejects the AHA-aligned path (AHA, September 2026).
ACLS Certification Association charges $275 initial and $175 renewal. The course is 100% online with no hands-on skills testing required. It includes 8 CME/CE credits (initial) or 4 CME/CE credits (renewal). Provider cards accepted in every US state and Canadian province; unlimited final exam retakes included.
American CPR Care Association charges $119 for both initial and renewal PALS certification. 100% online course; 4 CE credits included; valid 2 years. Accepted by some non-pediatric employers but typically NOT by pediatric ICUs or pediatric ERs (ACLS Cert Assoc, September 2026).

|---|---|---|---|---|---|
| AHA HeartCode + skills | $238-$288 | $238-$288 | Required ($60-$110) | 4-5 CEH | Yes (gold standard) |
| ACLS Cert Assoc | $275 | $175 | Not required | 8 / 4 CME | Not AHA, partial acceptance |
| American CPR Care | $119 | $119 | Not required | 4 CE | Not AHA, mostly non-pediatric |
| CPR VAM | $150-$280 | $150-$280 | Required | 2-3 CE | AHA-aligned |
| CPR Cart (HeartCode) | $155.75 + $110 skills | $155.75 + $110 skills | Required | 2-3 CE | Yes (HeartCode path) |
| ACLS.com (recert only) | n/a (recert) | $159 | Not required | 4 AMA Cat 1 | Online-only |
PALS renewal vs initial: same course, same price
As with ACLS, the AHA no longer differentiates between PALS initial and PALS renewal certification since 2020. The HeartCode PALS path treats every certification cycle the same: complete the online portion + complete the in-person skills session + receive the AHA PALS Provider eCard valid 2 years.
For training centers (UCSF, Idaho Medical Academy, Children's Minnesota), the AHA eliminated separate renewal courses. The recertification process is identical to initial certification: complete HeartCode PALS again, take the skills session again. The pricing is the same.
Private organizations still price renewal lower than initial. ACLS Cert Assoc renewal $175 vs $275 initial. American CPR Care is the same $119 for both. Most employers do not require separating initial and renewal cycles; the AHA-aligned standard is the 2-year renewal cycle.
For PALS specifically, renewal is critical for nurses in pediatric units where PALS skills atrophy faster (pediatric emergencies are high-stakes and infrequent). Most pediatric ICU and pediatric ER employers mandate PALS renewal every 2 years without exception (AHA, September 2026).
What PALS certification covers
PALS certification in 2026 covers pediatric-specific resuscitation skills distinct from ACLS:
- Pediatric assessment: Pediatric Assessment Triangle (PAT) for rapid initial impression; primary and secondary assessments.
- Respiratory emergencies: Upper airway obstruction, lower airway obstruction (asthma, bronchiolitis), respiratory distress vs failure.
- Cardiac emergencies: Cardiac arrest rhythms (asystole, VF, pulseless VT), symptomatic bradycardia and tachycardia, post-cardiac arrest care.
- Shock: Hypovolemic, distributive, cardiogenic, obstructive shock recognition and initial management.
- Megacode scenarios: Team-based simulated pediatric resuscitation with role assignment, closed-loop communication, and post-event debriefing.
- Medication dosing: Pediatric weight-based dosing for epinephrine, amiodarone, atropine, adenosine, sodium bicarbonate, calcium, dextrose.
PALS certification does NOT cover neonatal resuscitation (NRP), pediatric ICU procedures (central lines, intubation), or pediatric emergency procedures (chest tubes, needle decompression). These are separate specialty certifications. PALS is the foundational pediatric emergency certification required for pediatric inpatient and ER nurses (AHA, September 2026).
Total cost of PALS certification over a nursing career
Total cost of PALS certification over a typical 20-year pediatric nursing career in 2026: $2,000-$3,000 cumulative ($238-$288 every 2 years for AHA-aligned + $50-$100 study materials per renewal). This cost is typically funded by employer continuing education stipends ($500-$1,500/year at major hospital systems).
For pediatric ICU nurses, PALS renewal is non-negotiable every 2 years. Most pediatric hospital systems (Boston Children's, CHOP, Texas Children's, Seattle Children's, Cincinnati Children's) require PALS for all clinical staff regardless of role. The renewal cost is built into nursing benefits packages.
For travel pediatric nurses, PALS certification is required for almost all travel contracts in pediatric settings. Travel nursing agencies (AMN, Aya, Cross Country) typically fund PALS renewal ($238-$288) as part of sign-on bonuses ($1,000-$5,000 per assignment) (AHA, September 2026).
How PALS compares to PEARS, NRP, and ENPC
Multiple pediatric-specific emergency certifications exist in 2026. Choosing the right one depends on the nursing role and clinical setting.
PALS (Pediatric Advanced Life Support) is the AHA flagship pediatric emergency certification required for PICU, pediatric ER, and pediatric critical care nurses. Cost $238-$288 AHA-aligned. Validity 2 years. Covers full pediatric resuscitation including megacode.
PEARS (Pediatric Emergency Assessment, Recognition, and Stabilization) is the less intensive AHA certification for floor nurses, school nurses, and outpatient pediatric providers. Cost approximately $100-$150. Validity 2 years. Covers initial assessment and stabilization only; does NOT include advanced interventions (intubation, central line, etc.).
NRP (Neonatal Resuscitation Program) is required for NICU nurses, neonatal nurse practitioners, and delivery room staff. Cost $115-$175 for the online + skills check. Validity 2 years. Specialized for newborn resuscitation in the first hours of life.
ENPC (Emergency Nursing Pediatric Course) is offered by ENA (Emergency Nurses Association) for pediatric ER nurses. Cost $250-$400 in-person or $200-$300 online. Validity 4 years. Complementary to PALS; some employers require both PALS and ENPC for pediatric ER roles.
For most pediatric nurses, PALS is the foundational certification required. PEARS, NRP, and ENPC are role-specific additions (AHA, September 2026).
- The 2025-2026 AHA Guidelines update is in effect, covering pediatric resuscitation updates including post-cardiac arrest care and shock management. PALS courses updated to the 2025-2026 guidelines are now required for renewal.
- AI-powered PALS training (HeartCode's new AI simulation features launching late 2026) is expected to reduce online PALS prep time by 25-40%; AHA pricing likely unchanged for 2027.
- VR-integrated PALS skills training is being piloted at major children's hospitals; expect $30-$50 lower cost by 2027.
