The Certified Registered Central Service Technician (CRCST) credential is the entry-level certification for the technicians who sterilize every surgical instrument used in every hospital in the United States. The exam fee is the small part of the cost. The 400-hour hands-on experience requirement is the larger part. The recertification math — $50 a year forever, 12 CE credits a year, a 6-week wait between retakes if you fail — is the part nobody tells you about on day one. The Exam Fee, Exactly
The CRCST exam costs $140 for the initial attempt and $140 for each retake (HSPA 2026 Certification Handbook, myhspa.org). HSPA does not charge a separate application fee; the $140 covers submission and one Prometric appointment. The exam is 150 multiple-choice questions — 125 scored and 25 unscored pretest items — completed in 3 hours at a Prometric testing center.
The $140 retake number matters more than it looks. HSPA enforces a mandatory 6-week waiting period between attempts — stated on the official retake page, not just in third-party prep guides. The HSPA retake policy is hard. There is no appeal, no waiver for financial reasons, no acceleration during retake windows. The 6-week gap is to discourage rapid-fire retesting; candidates who fail need time to study, not just to retry.

Three domains — Cleaning, Preparation and Packaging, and Sterilization Process — together account for 63% of the exam. If you are deciding what to study first, that is the answer, and it comes straight off the published outline rather than from anyone's opinion.
HSPA does not publish a passing score. The cut score is set by a panel of subject matter experts using the Angoff method and approved by the Certification Council; candidates receive a pass or fail rather than a number. Any site that gives you a specific percentage is guessing.
The 400-Hour Rule
Full eligibility requires 400 hours of hands-on sterile processing experience within the previous 5 years, documented by a supervisor (HSPA 2026 Certification Handbook). The 400 hours are not one block. They are five separate minimums across the work:

Each function must hit its individual minimum before the total counts. Hours expire after 5 years from when they were earned; candidates who collected hours in a clinical rotation and waited too long to test find their numbers no longer count.
There is a Provisional Certification path for candidates who have not yet completed the 400 hours: you can sit the exam before completing them, and submit the hours within 6 months of passing (one-time 2-month extension available). The provisional option is the right choice for sponsored training programs that test students before graduation.
The Textbook and the Prep Programs

The textbook is not optional — the exam content draws from the manual and candidates without the manual are studying from second-hand sources. The community-college path is the most expensive upfront but bundles the exam fee and the 400-hour clinical placement. Candidates who are not already working in a sterile processing department cannot easily assemble the 400 hours on their own; the structured program is the path for career-changers. Recertification: The Math Nobody Tells You

Twelve CE credits per year is achievable through HSPA's own free or low-cost channels: lesson plans, webinars, and the Process This podcast. Each HSPA lesson plan earns 2 CE credits after the quiz; a single HSPA podcast earns 0.5 CE. The math is roughly 6 lessons + 6 podcasts = 12 CE credits, all free or covered by membership. Candidates who fall behind on CE credits and try to make them up at the end of the year run into the practical limit of how much content HSPA releases per quarter.
The ten-year cost of holding CRCST is roughly $500 in renewal fees alone, plus the time cost of the CE. Skip a renewal and the credential lapses; lapsed candidates must reapply and re-pay.
The CBSPD Alternative
The Certification Board for Sterile Processing and Distribution (CBSPD) offers the CSPDT credential. CBSPD eligibility requires either completion of an SPD training course with a passing grade of 70+, completion of 6 months of full-time SPD employment, or completion of 12 months of full-time SPD activities (CBSPD). CBSPD exam fee is $128. Recertification requires 100 CE points every 5 years.
CBSPD is the right alternative for candidates whose training pathway does not include the HSPA-sponsorship model. The CRCST is the more widely recognized credential; CBSPD is the practical alternative for career changers who cannot assemble HSPA's 400-hour requirement easily.
The Salary That Decides the Math
BLS and state-specific data put the sterile processing technician median salary in the $40,000–$48,000 range, with experienced technicians in high-cost metros reaching $55,000–$60,000. The credential itself does not raise salary by a fixed percentage; the credential unlocks jobs that require it, which carry higher pay than jobs that do not.
The DC government's 2026 sterile processing technician profile lists an average DC yearly salary of $56,640 with 40 projected annual openings. The job market is stable, not booming. Candidates choosing this credential for career stability rather than wage growth will get both; candidates expecting the credential to function as a salary booster will be disappointed without the additional 5+ years of experience and specialization.
What to Budget For
Three decisions sit below the CRCST cost question.
If you already work in an SPD department and have most of the 400 hours: budget $290 first year ($140 exam + $150 manual), then $50–$60 every year thereafter. Self-study is sufficient.
If you are a career-changer without SPD hours: budget $3,200+ for a structured program that includes clinical hours and the exam voucher. If you are deciding between CRCST and CBSPD: CRCST is the more recognized credential and the more useful one for hospital employment. CBSPD is the practical alternative when the HSPA 400-hour pathway is unavailable.
